1 In-network applies to both the Delta Dental PPO™ and Premier® networks.
2 Reasonable and Customary (R&C) amounts are based on the prevailing usual, actual, and community average charge as determined by Delta Dental. You’re responsible for paying the difference between what the provider charges and the R&C amount. This is in addition to your coinsurance and any amounts above the annual benefit maximum, if applicable.
3 If you satisfy the $2,000 orthodontia lifetime maximum under the Comprehensive Dental option and then enroll in the Enhanced Dental option, you will not be eligible for the additional $500 lifetime maximum.
4 Amounts accumulated cross apply for both in-network and out-of-network benefits up to $2,000; under the Enhanced option, an additional $2,000 of in-network benefits is available toward the Annual Benefit Maximum.

Deductible
Amount you pay for non-preventive covered services before your plan begins to pay.
Lifetime Maximum
A lifetime maximum is the maximum amount of money a company agrees to pay for covered services during an individual’s lifetime. Amounts accumulated toward any lifetime maximums under the plan are carried over from year to year, regardless of whether you change coverage options during Annual Enrollment.
Annual Benefit Maximum
The annual benefit maximum is the most the Pfizer dental plan will pay in a calendar year for each covered individual and applies to all covered services, except orthodontia, which has a separate lifetime maximum. Also, amounts accumulated toward the orthodontia lifetime maximum are carried over from year to year, regardless of whether you change coverage options during Annual Enrollment.
Consider these when choosing your dental option:
- Dental Services You’re Planning: If you know the dental services you and/or your family need next year (e.g., major services, orthodontia), talk with your in-network provider about the costs for those services. If the provider’s fees exceed the annual benefit maximum under the Comprehensive Dental option, determine if it’s better to enroll in the Enhanced Dental option with its higher in-network annual benefit maximum.
- Dental Contributions & Preventive Services: Consider the contribution amounts you pay for each option and the preventive services the you will need (i.e., the number of annual preventive exams and/or cleanings) as they are different for each option. Comprehensive Dental covers two cleanings and/or exams with lower contributions. Enhanced Dental covers three cleanings and/or exams with higher contributions.
- Out-of-Network Dental Coverage: If you will be using out-of-network dental providers, you may want to consider the Comprehensive Dental option since it offers the same level of coverage for out-of-network services as the Enhanced Dental option without the $50 out-of-network deductible.
