2027 Health and Insurance Changes

This is a summary of important changes affecting your health and insurance coverage options. For more details and additional changes, review the Summary of 2027 Benefit Changes.

Medical

Deductibles and/or out-of-pocket maximums are increasing for some medical plan options. For out-of-network services, the basis for determining the maximum allowed amount for those services is changing. See the Summary of 2027 Benefit Changes for more information.

See the Medical Plan Comparison Chart to compare the available plan options.

2027 Contributions

For 2027, there will be increases to medical contributions. Contributions for dental and vision remain unchanged. Rates will also be increasing for LTD coverage. Remember, contributions for Supplemental Life, AD&D and LTD may also change year to year if your covered pay changes or if you reach a new age band (for life insurance).

Prescription Drug

The prescription drug per prescription minimums and maximums for non-Pfizer brand medications and Pfizer medications (when a generic is available) are increasing. The out-of-pocket maximums are also increasing.

The copay for non-Pfizer biosimilar medications is increasing. If you continue to fill the reference product for the biosimilar medication without a prior authorization through CVS Caremark, you will be required to pay your regular coinsurance plus an additional fee of $500 per 30-day supply (this additional fee will not count toward your deductible or out-of-pocket maximum). For 2027, this will affect members taking Epogen, Procrit, Prolia and Remicade; these will be in addition to Humira and Stelara.

  • Eligible Pfizer biosimilar medications dispensed through a pharmacy remain at no cost to you (unless you are enrolled in the HSA Copay option, then they will be covered at no cost after your deductible is met, unless they are on the HSA Preventive Drug List).

Coverage for Pfizer’s Xeljanz and certain forms of Premarin medications will be changing. Effective January 1, 2027, these medications will no longer be covered at no cost. Now that generics are available, these medications will be covered as a Pfizer Medication (when a generic is available) and you will pay the applicable cost share under your Pfizer prescription drug coverage. Review the Zero Cost Prescription Drug List.

See the Prescription Drug Summary Chart.

Health Savings Accounts

If you elect the HSA Copay medical plan option, most colleagues are eligible to receive a Company contribution to their Pfizer HSA. For 2027, the pay bands for the Pfizer funding are increasing and the amount Pfizer contributes for the Less than $90,000 pay band is increasing as well. The following changes will apply.

The annual maximum HSA contribution set by the IRS will increase:

  • Individual coverage — from $4,400 to $4,500
  • Family coverage — from $8,750 to $9,000

Keep in mind, if you will be at least age 55 during 2027, you may contribute an additional $1,000 to your HSA as an HSA catch-up contribution.

Note: The annual HSA contribution limits noted above include your contributions plus Pfizer’s Company contribution, if applicable. Remember, the HSA Copay option is the only option that includes the ability to contribute to a Health Savings Account—and receive a Company contribution from Pfizer if you are eligible depending on your annual base pay.

Limited Purpose (LPHCA) and General Purpose (GPHCA) Health Care Accounts

If you elect the 2026 maximum contribution limit of $3,400, your 2027 LPHCA/GPHCA election will automatically increase to the 2027 limit after it is announced by the IRS. Remember, this election may not be changed mid-year. Refer to the Health Care Accounts Summary for more information.

Long-Term Disability

Contributions are increasing for all options with the highest increase for the 70% option; the 50% option continues to be provided at no cost. To learn more about the different LTD options and evaluate your coverage, review the Long-Term Disability Video Overview and attend a webinar through the Virtual Annual Enrollment Benefits Fair to learn more.

Supplemental Life Insurance

During the 2027 Annual Enrollment period ONLY, the amount of Supplemental Life Insurance you can obtain without providing Proof of Good Health is increasing. You may elect or increase your supplemental life insurance coverage up to 3x annual pay, up to $1 million, without Proof of Good Health, regardless of whether you have no coverage, previously failed Proof of Good Health or have less than 3x annual pay (up to $1 million).

Remember, if enrolled in Supplemental Life for you or your spouse/domestic partner, your contributions are based on age as of December 31, 2027, and may increase due to increases in pay and/or moving to a higher age bracket (rates increase every five years, e.g., age 25, 30, 35, 40, 45, etc.).
