Your nurse can also connect you with other Pfizer benefits and resources that may be available.

Medication Management for Chronic Conditions
The Pharmacy Advisor® Program offers personalized prescription drug support if you’re diagnosed with a chronic condition such as asthma, diabetes, or high blood pressure. A pharmacist will help you stay on track with your medication and understand how it works. He or she can also help you sign up for automatic refills and understand the Maintenance Choice Program when you use a CVS Pharmacy or the CVS Caremark Mail Service Pharmacy to help you save time and money.

Maternity Support Program through Optum (for Horizon and UHC members)
The Maternity Support Program, administered by Optum, provides a dedicated registered maternity nurse who will support you and your baby before, during and after pregnancy. When you register for the program (recommended to be during the first trimester), you’ll receive important materials about a healthy diet, prenatal care, exercise, and how your baby grows. If your pregnancy is considered high-risk, you also have access to specialized support services, including an obstetric nurse who will work closely with your OB/GYN throughout your pregnancy to provide answers, support, and guidance. Download the Optum Healthy Beginnings app for a one-click connection to a nurse 24/7 and access personalized content about your pregnancy based on your due date.
Orthopedic Health Support through Optum (for Horizon and UHC members)
Orthopedic Health Support can help you assess your treatment options (surgical and non-surgical) and locate providers for musculoskeletal (knee, hip, joint or spine) conditions. If you need surgery, this program can help you understand your benefits, prepare for surgery, and provide support as you recover. If you’re enrolled in a Pfizer Medical Plan through UHC, your knee, hip, or spine surgery may be covered at 100 percent (after deductible) when you participate in this program and you use certain high-quality providers.
Fertility Solutions through Optum (for Horizon and UHC members)
The Fertility Solutions program provides access to registered nurses who specialize in family building. These nurses can provide information about treatment options and help you understand how your coverage works.
Participation in Fertility Solutions is required to receive benefit coverage and have eligible family building treatment claims paid through the Pfizer Medical Plan (for both medical and prescription medication costs), up to a lifetime maximum of $25,000. To help improve the likelihood of a successful pregnancy, Fertility Solutions can help you find doctors and other health care professionals who are experts in their field and are part of the Centers of Excellence (COE) if you are enrolled with UnitedHealthcare. Additionally, if you use a COE, as applicable, you will qualify for an additional $10,000 toward your lifetime maximum in eligible benefit coverage; in that case, your lifetime maximum will be $35,000. Eligible covered expenses to diagnose infertility are not applied toward the lifetime maximum.

Viewing Your Lifetime Family Building Maximum
View your combined medical and prescription drug lifetime maximum for family building treatment on caremark.com. Log on and access the Plan Summary under the Plan & Benefits tab and view the Family Building Maximum Allowable Benefit (MAB) (amounts are updated monthly; contact CVS Caremark for the current amount accumulated toward your lifetime maximum). Amounts accumulated toward the lifetime maximum are carried over from year-to-year, regardless of whether you change coverage options or medical plan administrators.
