Health and Insurance Confirmation Statements: By mid-November, you will receive a statement at your home address on file from the Pfizer Benefits Center confirming your 2025 health and insurance elections. The last day you can make changes or corrections is December 6.

Benefit Extras Confirmation Email: Once you submit your elections, you will receive an email confirming your elections. You can also review your elections by visiting PfizerBenefitExtras.com at any time. The last day you can make changes or corrections is December 6, except for Life with Long Term Care (LTC), which you can change through December 4.

If You Don’t Enroll

If you don’t enroll, your current elections will continue for 2025, with a few exceptions:

  • You will not participate in the Limited Purpose or General Purpose Health Care Account or the Dependent Care Account. To participate for 2025, you must actively enroll and elect an annual contribution amount. Reminder, unused amounts (for the Health Care Account) above the IRS carryover limits are subject to the IRS “use it or lose it” rule.
  • You will not be able to purchase additional vacation for 2025. Remember, purchased vacation must be used for the year it’s elected and is not eligible to be carried over. Any unused vacation purchased for 2025 will be refunded as taxable pay by December 31, 2025. Information about the payout process is communicated in November of each year.
  • If you are age 65 or older and enrolled in the HSA Copay option, you will be automatically opted out of the HSA contribution election. You need to call the Pfizer Benefits Center by December 6, 2024 to confirm you meet the eligibility requirements and enroll. For all other colleagues, your 2024 HSA contribution election will continue for 2025 if you remain enrolled in the HSA Copay option.

Your benefits elections will take effect on January 1, 2025, and will remain in effect through December 31, 2025. You can only make changes during the year if you experience a qualifying event, so make your enrollment decisions carefully. Different rules may apply if you are on leave of absence and you do not return by January 1, 2025. Contact the Pfizer Benefits Center at 1-866-476-8723 to understand how your 2025 elections are affected by your leave of absence.

Important Annual Enrollment Considerations

Annual Enrollment is the only time you can take certain actions on your benefit programs.

Life and Disability Insurance

  • Reduce your basic life insurance to $50,000 to avoid imputed income (or increase it back to 1x annual pay — Proof of Good Health will be required).
  • Elect to increase Supplemental Life Insurance for yourself by 1x annual pay (up to $1,000,000), without providing Proof of Good Health. Proof of Good Health will only be required if you are electing this coverage for the first time, previously did not meet the Proof of Good Health requirements, if this coverage is greater than $1,000,000 (regardless of your current coverage amount), or if you are electing an increase of more than 1x your annual pay.
  • Elect or increase spouse/domestic partner life insurance coverage (up to $250,000). You may elect coverage equal to your own total life insurance coverage (basic and supplemental), not to exceed $250,000. Proof of Good Health is required if you are electing coverage for the first time or increasing coverage regardless of the amount.
  • Update the smoker status for you and/or your spouse/domestic partner.
  • Change your long-term disability coverage.

Proof of Good Health for the Pfizer Life Insurance Plan

If you elect life insurance coverage that requires Proof of Good Health, you will be prompted during the enrollment process on next steps and timing. Once MetLife, the Plan insurer, approves your Proof of Good Health, your full, elected coverage amount will go into effect. Until then, you will have the maximum amount of coverage allowed without Proof of Good Health.

Benefit Extras

During Annual Enrollment, you can enroll in:

  • Accident Insurance, Critical Illness Insurance, and/or Hospital Indemnity Insurance — once enrolled, you can cancel any time.
  • Legal Services — once enrolled, you may only cancel your participation during Annual Enrollment.
  • Life Insurance with Long Term Care — if you were hired or newly eligible between October 1, 2023 and September 30, 2024, you may be permitted to enroll in up to $200,000 of Life Insurance with $400,000 in Long Term Care coverage without Proof of Good Health; otherwise, you will be required to provide Proof of Good Health.

Taking a Leave of Absence

If your Leave of Absence begins on or before September 13, 2024, enrollment information will be mailed to your home. If your Leave begins after September 13, call the Pfizer Benefits Center at 1-866-476-8723 to understand how your elections may be impacted.

Enrollment Information for COBRA Coverage for Children up to Age 30

If your child turned age 26 and is continuing coverage under COBRA, they will receive a separate enrollment kit mailed to their home address on file. If they have questions about making enrollment changes during the Annual Enrollment period, they should call the Pfizer Benefits Center. If your child is turning age 26 before January 1, 2025, you will receive more information about 60 days prior to their 26th birthday about how to continue their coverage.
